We are seeking a full-time, fully remote RN Care Guide to join our Shared Services team, with a strong focus on Complex Case Management and telephonic clinical support.<br><br>This role supports patients through RN triage, disease education, care planning, care coordination, transition of care management, medication education, ED diversion, and barrier resolution.
