Identifies and analyzes patient care issues, redesigns processes, and implements evidence-based practices. Conducts comprehensive chart reviews to monitor compliance with quality indicators and identify areas for improvement. Gathers, analyzes, and reports data related to performance and project objectives to inform decision-making. Facilitates meetings, records proceedings, and directs committees to address improvement and educational opportunities. Coordinates and implements improvement strategies, ensuring alignment with best practice standards. Collaborates with healthcare providers and staff to develop and execute quality improvement plans. Tracks and monitors quality metrics, providing regular updates to relevant stakeholders. Prepares and presents reports on quality indicators, safety, mortality, readmissions, and other required measures. Engages in data collection and analysis for publicly reported quality indicators and external benchmarking programs. Provides training and education to staff on quality improvement protocols and best practices. Integrates internal and external benchmarks to continuously improve clinical and operational performance. Other duties as assigned. Knowledge, Skills, and Abilities: • Excellent computer skills, including advanced use of Microsoft Word, Excel, and PowerPoint. [Required] • Experience in complex chart reviews, running reports, analyzing data, working with data systems, and ability to manipulate data in multiple modalities such as Excel. [Required] • Experience preparing professional presentations to describe data, results, and analysis to various clinical audiences, including physicians, nursing, pharmacy, and specialty staff. [Required] • Experience in navigating and interpreting the medical literature and identifying articles that summarize best practices. [Required] • Experience in implementing evidence-base guidelines into clinical practice, using process improvement methodologies, data, clinical background, knowledge, and the medical literature. [Required] • Ability to work with an electronic health record. [Required] • Ability to effectively and efficiently coordinate multiple priorities and issues to facilitate data and quality management needs. [Required] • Possess strong communication skills and excellent organizational skills. Ability to maintain and interact in a professional manner. [Required] • Ability to function as part of a team and develop strong multidisciplinary relationships and partnerships.
