This role also involves working directly with external providers such as PCPs<br>and NPs to ensure holistic, seamless, high-quality care for our diverse patient<br>population.</p> <p><br>The Clinical Care Manager is responsible for improving both patient care and the<br>patient experience while managing and supporting the care team’s operations. This is a<br>largely virtual outpatient multidisciplinary care model that we are learning to scale,<br>and this role must be able to thrive working in a fast-paced, often ambiguous,<br>high-patient-touch outpatient environment that includes phone calls, text messages,<br>virtual visits, and community visits with operational elements still being defined. This<br>role requires compassion, creativity, innovative problem-solving, and the ability to work<br>collaboratively with the care teams to ensure high-quality, patient-centered care.</p> <p> </p> <p>Responsibilities:</p> <p><br>Clinical Support & Coordination:</p> <p><br>● Oversee the daily activities of assigned multidisciplinary care teams,<br>ensuring effective collaboration and seamless care delivery.<br>● Provide clinical expertise and support to team members, particularly in<br>managing complex cases and clinical decisions.<br>● Lead clinical triaging efforts, ensuring appropriate escalation to maintain<br>patient safety and care quality.<br>● Enhance the clinical effectiveness of the teams, ensuring adherence to the<br>Vayu Care Model and best practices.<br>● Ensure adherence to HIPAA compliance regulations to protect patient<br>privacy and confidentiality.<br>Team Management & Development:<br>● Oversee all aspects of team management, including recruitment,<br>onboarding, training, coaching, performance evaluations, disciplinary<br>actions, and team transitions while fostering a collaborative,<br>high-performing environment focused on delivering exceptional patient<br>care.<br>● Actively engage with your team in daily operations, collaborating closely<br>with them to solve challenges, provide clinical guidance, and ensure<br>efficient workflows.<br>● Create an environment that encourages continuous learning, professional<br>development, and open communication within the care team.<br>● Build and maintain strong working relationships with external clinic staff,<br>community clinics/federally qualified health centers (FQHCs), and providers<br>to support a collaborative, patient-centered approach to care.<br>● Facilitate regular huddles and case conferences to address challenges and<br>improve team morale and productivity.</p> <p>Operational Excellence:</p> <p>● Help optimize workflows, schedules, and clinical documentation to improve<br>team effectiveness and ensure efficient, seamless patient care.<br>● Help monitor and improve key clinical effectiveness metrics, and develop<br>actionable plans to address any performance gaps.<br>● Ensure compliance with HIPAA, regulatory standards, and clinical care<br>contracts, particularly as they pertain to California regulations.</p> <p>● Problem-solve and multitask effectively to adapt effective human centered<br>care delivery to meet the evolving needs of both patients and the care<br>team.</p> <p>Strategic Growth & Continuous Improvement:</p> <p>● Contribute to the development and implementation of clinical projects that<br>drive improvement in both patient care and team performance.<br>● Support the adoption of technology and EMR systems that enhance care<br>delivery and streamline workflows as well as be a superuser.<br>● Identify opportunities to adjust team roles and responsibilities to ensure<br>continued alignment with Vayu Health’s care model and strategic goals as<br>we grow.<br>● Help implement and recruit members into our program.<br>● Performs other duties as assigned.</p> <p><br>Required Qualifications:</p> <p>○ Bachelor of Science in Nursing (BSN) or other applicable field.<br>○ Active and unencumbered RN or LCSW or LMFT license in the state of California.<br>○ Experience working with underserved populations.<br>○ Minimum of 5 years of clinical nursing experience, with demonstrated<br>proficiency in triaging and managing complex patient cases.<br>○ Minimum of 3 years of comprehensive supervisory experience,<br>showcasing the ability to effectively lead, mentor, and motivate a<br>multidisciplinary clinical team to achieve high-performance standards.<br>○ In-depth knowledge of regulatory compliance in healthcare, with a<br>focus on California regulations.<br>○ Excellent communication, conflict resolution, and team-building skills,<br>with a commitment to fostering a collaborative and patient-centered<br>work environment.</p> <p>Preferred Qualifications:</p> <p>● Fluency in both English and Spanish<br>● Lived experience<br>● Experience in design and/or improvement work<br>● Experience volunteering in your community<br>● Known for thinking outside the box to improve member care</p>
